Description
Milam is indicated for short-term treatment of insomnia and sedation in premedication before surgical or diagnostic procedures. It is a sleep-inducing agent characterized by a rapid onset and short duration of action. It also exerts an anxiolytic, anticonvulsant, and muscle-relaxant effect.

Mode of Action
The actions of benzodiazepines such as midazolam are mediated through the inhibitory neurotransmitter gamma-aminobutyric acid (GABA), resulting in a sedating effect, relaxation of skeletal muscles, and induction of sleep, anesthesia, and amnesia.
Dosage & Administration
- Oral dosage for adults: 7.5-15 mg daily
- Recommended oral dose for elderly and debilitated patients: 7.5 mg
- Intravenous administration for endoscopic or cardiovascular procedures: initial dose in healthy adults is approximately 2.5 mg
- Intravenous administration for induction of anesthesia: 10-15 mg
- Intramuscular administration for adults: 0.07-0.1 mg/kg body weight
- Rectal administration in children for preoperative sedation: 0.35-0.45 mg/kg
Interaction
Milam can enhance the central sedative effect of various drugs including neuroleptics, tranquillizers, and antipsychotics.
Contraindications
- Severe respiratory insufficiency
- Severe hepatic insufficiency
- Myasthenia gravis
- Sleep apnea syndrome
- Known hypersensitivity to benzodiazepines or any component of the product
Side Effects
- At the start of therapy: drowsiness, confusion, fatigue, headache, muscle weakness
- Following parenteral administration: respiratory depression, apnea, variations in blood pressure and pulse rate
Pregnancy & Lactation
Midazolam should be avoided during pregnancy unless there is no safer alternative. It passes into breast milk, so it should not be administered to breastfeeding mothers.
Precautions & Warnings
Milam IV should be administered very slowly.
Overdose Effects
Extreme overdosage may lead to coma, areflexia, cardiorespiratory depression, and apnea. Overdosage effects can be controlled with the benzodiazepine antagonist flumazenil.
