Indications

Description

Pharmacology
- attaches to specific site on GABA receptor
- potentiates the effect of GABA
- rapid and complete absorption after oral administration
- peak plasma concentration within 15-90 minutes
- mean plasma half-life of 30 hours
- plasma protein binding of 98-99%
- metabolized in the liver
- small proportion excreted in urine
- metabolites excreted through the bile into the intestine
- eliminated with the feces
- significant absorption after rectal administration
- peak concentration within 1.5-2 hours
Dosage & Administration
- 2 mg thrice daily for anxiety
- 15-30 mg daily in divided doses for anxiety
- dose adjustment for elderly or debilitated patients
- 5-15 mg at bedtime for insomnia associated with anxiety
- 1-5 mg at bedtime for night terrors in children
- 10 mg repeated if necessary for severe acute anxiety
- 250 mcg/kg for febrile convulsion in children
- 500 mcg/kg (max 10 mg) for rectal administration in children
Interaction
- not recommended with alcohol
- increased sedation with concomitant use of neuroleptics and hypnotics
- decreased clearance with phonobarbitone
- inhibited metabolism with omeprazole and isoniazide
Contraindications
- myasthenia gravis
- pulmonary insufficiency
- respiratory depression
- hypersensitivity to benzodiazepine

- crosses placental barrier
- passes into breast milk
- avoid if possible during pregnancy and lactation

Precautions & Warnings

- use with caution in respiratory disease
- use with caution in muscle weakness
- use with caution in history of drug or alcohol abuse
- use with caution in hepatic or renal impairment
Overdose Effects
- sedation
- muscle weakness
- profound sleep or paradoxical excitation
- ataxia
- hypotonia
- hypotension
- respiratory depression
- rarely coma and death
