Indications

  • Treatment of benign prostatic hyperplasia (BPH)
  • Treatment of lower urinary tract symptoms (LUTS)
  • Relief of urinary frequency
  • Relief of nocturia
  • Relief of incomplete emptying
  • Relief of urinary hesitancy

Pharmacology

  • Selective antagonist of post-synaptic a1 adrenoreceptors
  • Relaxes tone of prostate smooth muscle
  • Relaxes prostate capsule
  • Relaxes bladder neck
  • Relaxes proximal urethra
  • Binds competitively and selectively to post synaptic a1-adrenergic receptors

Dosage

  • Recommended dose of 10 mg once daily after a meal for BPH
  • Recommended dose of 10 mg daily after a meal for acute urinary retention (AUR) in patients 65 years and older

Administration

  • Tablet should be swallowed whole

Interaction

  • Contraindicated with alpha-1-receptor blockers
  • To be cautious with antihypertensive drugs
  • To be cautious with nitrates
  • Potent CYP3A4 inhibitors to be taken into account

Contraindications

  • Use with caution in patients with acute cardiac failure
  • Caution in patients with hypersensitivity to alpha-1-blockers
  • Caution in patients with congenital or acquired QTc prolongation
  • Not recommended for patients with severe renal impairment
  • Avoid other modes of administration such as crushing or grinding
  • Inform ophthalmologists of past use of alpha-1-blockers before cataract surgery

Side Effects

  • Common: faintness/dizziness, headache
  • Uncommon: syncope, vertigo, malaise, drowsiness
  • Uncommon: vision abnormal
  • Uncommon: tachycardia, palpitations
  • Uncommon: hypotension (postural)
  • Common: nausea, abdominal pain
  • Uncommon: diarrhoea, dry mouth, vomiting
  • Uncommon: rash, pruritus
  • Common: asthenia, flushes, chest pain

Pregnancy & Lactation

  • Not applicable due to indication

Precautions & Warnings

  • Withdraw tablets before general anesthesia
  • Discontinue if symptoms of angina pectoris start or worsen

Use in Special Populations

  • Not indicated for use in the pediatric population (2 to 16 years)

Overdose Effects

  • Hospitalization may be necessary
  • Supine position for the patient
  • Conventional treatment of hypotension
  • Corrective treatment may be vasoconstrictor
