Indications

• Acute anxiety treatment
• Chronic anxiety treatment
• Sleep disturbances
• Emotional instability
• Adjunctive therapy for depression
• Adjunctive therapy for epilepsy

Pharmacology

Binds to GABA receptors in CNS, increases chloride ionopore opening time, enhances GABA inhibitory effect

Dosage

• Adults and adolescents over 15 years: Initial dose of 20mg daily, maximum daily dose of 30mg
• Elderly: Lower initial doses, maintenance dose of 10-15mg daily
• Children from 3 to 15 years: 5-10mg daily, careful assessment needed
• Duration of treatment: As short as possible, not exceeding 8 to 12 weeks, assessment after 4 weeks

Administration

• Can be administered whole or crushed
• Can be mixed in applesauce
• Can be taken with or without food

Interaction

• Alcohol: Increases bioavailability by 50%
• Central nervous system depressant drugs: Mutually potentiating effect
• Opioids: Increases risk of sedation and respiratory depression
• Anticonvulsants: Possible interactions with basic anticonvulsant medication
• CYP 2C19 inhibitors: May result in increased exposure to active metabolite
• CYP 2D6 substrates: Weak CYP2D6 inhibitor

Contraindications

• Hypersensitivity to clobazam or excipients
• Myasthenia gravis
• Severe respiratory insufficiency
• Sleep apnoea syndrome
• Severe hepatic impairment
• Breastfeeding
• Children between 6 months and 3 years

Pregnancy & Lactation

• Pregnancy: Not recommended during first trimester, potential benefit justifies potential risk
• Lactation: Must not be used in breastfeeding women

Precautions & Warnings

• Serious Skin Reactions: Closely monitor for signs of SJS/TEN
• Respiratory depression: Monitor respiratory function, contraindicated in severe respiratory insufficiency
• Muscle weakness: Contraindicated in myasthenia gravis
• Renal and hepatic impairment: Increased responsiveness and susceptibility, regularly check function
• Elderly patients: Increased risk of adverse reactions, recommend dose reduction
• Tolerance in epilepsy: Consider possibility of tolerance development
• Suicidality: Reports of suicidal behavior, especially with confounding factors
• Concomitant use of CYP2C19 inhibitors: May result in increased exposure to active metabolite
