Pharmacology
- Binds to chloride ionopore at GABA receptor
- Increases duration of chloride ionopore opening
- Stabilizes post-synaptic membrane
- Enhances inhibitory effect of GABA
Dosage
- Initial dose: 20mg/day for adults
- Maintenance dose: 10-30mg/day for adults
- Initial dose: 5-10mg/day for children (3-15 years)
- Maintenance dose: 5-10mg/day for children (3-15 years)
- Max dose: 80mg/day for epilepsy treatment
- Dose adjustment after symptom improvement
- Treatment duration: maximum 8-12 weeks
- Gradual dose reduction for discontinuation
Administration
- Can be administered whole or crushed
- Can be mixed in applesauce
- 10mg tablets can be divided into equal halves
- Can be taken with or without food
Interaction
- Increased bioavailability with alcohol consumption
- Potentiation with CNS depressant drugs
- Risk of sedation, respiratory depression with opioids
- Potential interactions with anticonvulsants
- Enhanced euphoria with narcotic analgesics
- Possible enhancement of muscle relaxants and nitrous oxide effects
- CYP 2C19 and CYP 2D6 inhibitor interactions
Contraindications
- Hypersensitivity to clobazam or excipients
- Myasthenia gravis
- Severe respiratory insufficiency
- Sleep apnoea syndrome
- Severe impairment of liver function
- Breastfeeding
- Children aged 6 months to 3 years

- Decreased appetite
- Irritability, aggression, restlessness
- Sedation, dizziness, disturbance in attention
- Headache, tremor, ataxia
- Dry mouth, nausea, constipation
- Rash, photosensitivity reaction
- Muscle spasms, weakness
- Fatigue, slow response to stimuli
Pregnancy & Lactation
- Not recommended during 1st trimester
- Limited data on exposed pregnancies
- Potential risk of respiratory depression in newborns
- Cautious use in breastfeeding women

- Monitoring for serious skin reactions and respiratory depression
- Risk of muscle weakness in myasthenia gravis patients
- Increased susceptibility in renal and hepatic impairment
