Indications

  • Treats acute and chronic anxiety states
  • Relieves symptoms of anxiety, tension, restlessness, excitement, irritability, and sleep disturbances
  • Addresses psychovegetative and psychosomatic disorders
  • Adjunctive therapy for patients with depression or anxiety associated with depression
  • Adjunctive therapy for patients with schizophrenia or other psychotic illnesses
  • Functional for patients with epilepsy who are not adequately stabilized with their anticonvulsant monotherapy

Pharmacology

  • Binds at distinct binding sites associated with the chloride ionopore at the post-synaptic GABA receptor
  • Increases the duration of time for which the chloride ionopore is open
  • Enhances the post-synaptic inhibitory effect of GABA
  • Leads to hyperpolarization and stabilization of the membrane
  • Operates in various locations in the CNS (limbic, reticular formation)

Dosage

  • Initial dose usually 20 mg clobazam daily for adults and adolescents over 15 years of age
  • Maintenance dose of 10 to 15 mg clobazam daily for elderly patients
  • Daily dose of 5 to 10 mg clobazam for children from 3 to 15 years of age
  • Gradual dose increments under careful observation for elderly and pediatric patients
  • Timing of doses including larger portion taken in the evening
  • Duration of treatment must be as short as possible, not exceeding 8 to 12 weeks
  • Treatment must not be extended without re-evaluation of the patient's status using special expertise

Administration

  • Tablets can be administered whole or crushed and mixed in applesauce
  • 10 mg tablets can be divided into equal halves of 5 mg
  • Can be given with or without food

Interaction

  • Concomitant consumption of alcohol can increase the bioavailability of Cosium by 50%
  • Mutually potentiating effect with central nervous system depressant drugs when administered in higher doses
  • Concomitant use with opioids increases the risk of sedation, respiratory depression, and coma
  • Dosage adjustment necessary when administered simultaneously with anticonvulsants in the treatment of epilepsy
  • Possible euphoria enhancement if used concomitantly with narcotic analgesics
  • Effects of muscle relaxants and nitrous oxide may be enhanced

Contraindications

  • Hypersensitivity to clobazam or any of the excipients of Clobazam
  • Myasthenia gravis
  • Severe respiratory insufciency
  • Sleep apnoea syndrome
  • Severe impairment of liver function
  • Breastfeeding women
  • Children between the ages of 6 months and 3 years, except in exceptional cases for anticonvulsant treatment where there is a compelling indication

Side Effects

  • Decreased appetite
  • Irritability, aggression, restlessness, depression, drug tolerance, agitation
  • Somnolence, sedation, dizziness, disturbance in attention, slow speech, dysarthria
  • Headache, tremor, ataxia
  • Dry mouth, nausea, constipation
  • Rash, photosensitivity reaction, urticaria
  • Muscle spasms, muscle weakness
  • Fatigue, slow response to stimuli, hypothermia

Pregnancy & Lactation

  • Not recommended during the first trimester of pregnancy
  • Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus
  • Must not be used in breastfeeding women
  • Clobazam crosses the placenta
  • Neonatal respiratory depression, sedation signs, hypothermia, and hypotonia may occur in the newborn

Precautions & Warnings

  • Closely monitor for signs or symptoms of serious skin reactions, especially during the first 8 weeks of treatment
  • Respiratory function must be monitored especially in patients with chronic or acute respiratory insufficiency
  • Cautious use in patients with myasthenia gravis
  • Increased responsiveness and susceptibility to adverse effects in patients with impairment of renal or hepatic function
  • Recommended dose reduction in elderly patients
  • Consideration for the possibility of a decrease in anticonvulsant efficacy in the course of treatment
  • Increased risk of fall in elderly patients
