Indications
- Acute and chronic anxiety states
- Emotional instability
- Depression
- Anxiety associated with depression
- Psychotic illnesses
- Adjunctive therapy in epilepsy
Pharmacology
- Binds at distinct binding sites associated with the chloride ionopore
- Increases the duration of time for which the chloride ionopore is open
- Enhances post-synaptic inhibitory effect of GABA
Dosage
- Adjust dosage and duration of treatment based on indication and clinical response
- Initial dose for adults and adolescents is usually 20 mg daily
- Maintenance dose for elderly is 10 to 15 mg daily
- Children's dosage is 5 to 10 mg daily
- Timing of doses - larger portion recommended in the evening
- Duration of treatment not to exceed 8-12 weeks
Administration
- Tablets can be administered whole or crushed and mixed in applesauce
- 10 mg tablets can be divided into equal halves of 5 mg
- Can be taken with or without food
Interaction
- Increased bioavailability with alcohol consumption
- Potentiating effect with CNS depressant drugs
- Risk of respiratory depression with opioids
- Adjust dosage with anticonvulsants
- Enhanced muscle relaxants effects
- Inhibitors of CYP2C19 may increase exposure to active metabolite
Contraindications
- Hypersensitivity to clobazam or excipients
- Myasthenia gravis
- Severe respiratory insufficiency
- Severe liver function impairment
- Breastfeeding
- Not recommended for children under 3 years

- Irritability, aggression, restlessness
- Somnolence, sedation, dizziness

Pregnancy & Lactation
- Not recommended during first trimester
- Crosses placenta
- Potential fetal risk
- Not recommended in breastfeeding women

- Serious skin reactions like SJS/TEN
- Respiratory depression in high doses
- Muscle weakness
- Adjust dosage in renal and hepatic impairment
- Risk of fall in elderly patients
- Consideration of tolerance in epilepsy
- Increased risk of suicide
- Concomitant use of CYP2C19 inhibitors
