Indications
- Treatment for schizophrenia in patients unresponsive to conventional antipsychotic drugs
- Treatment for psychosis in Parkinson's disease
Pharmacology
- Atypical antipsychotic drug
- Binds to dopamine receptors
- Antagonist at adrenergic, cholinergic, histaminergic, and serotonergic receptors
Dosage & Administration
- Initial dose of 12.5 mg for adults with schizophrenia
- Initial dose of 12.5 mg at bedtime for adults with psychosis in Parkinson's disease
- Maximum dosage of 300 mg daily for schizophrenia
- Usual dose range of 25-37.5 mg at bedtime for psychosis in Parkinson's disease
Interaction
- Possible aggravation of myelosuppression with concomitant use of myelosuppressants
- Potential interaction with other CNS active drugs or alcohol
- Risk of orthostatic hypotension with benzodiazepine usage or other psychotropics
Contraindications
- Severe cardiac disorders
- Renal impairment
- History of neutropenia or agranulocytosis
- History of circulatory collapse
- Uncontrolled epilepsy
- Breastfeeding
Side Effects
- Common side effects include constipation, dizziness, drowsiness, headache, nausea, and vomiting
- Less common side effects include abdominal discomfort, heartburn, and dry mouth
Pregnancy & Lactation
- Limited studies in pregnant women
- Can be used in pregnancy if necessary
- Should not be used while nursing infants
Precautions & Warnings
- Tapering off medication is required
- Elderly patients are more susceptible to adverse effects
- Significant risk of agranulocytosis
- Weekly monitoring of WBC for the first 6 months of therapy
- Caution in patients at risk of seizures
- Potential for extrapyramidal symptoms appear to be extremely low
Overdose Effects
- Signs and symptoms include altered states of consciousness, tachycardia, hypotension, respiratory depression, and coma
- Seizures have occurred in a minority of reported cases
- Management includes established airway, adequate oxygenation and ventilation
