Indications
- Acute and maintenance treatment of schizophrenia and related psychoses
- Treatment of acute manic or mixed episodes in bipolar disorder
Pharmacology
- Antipsychotic agent with affinities for serotonin and dopamine receptors
- Well absorbed after oral administration
- Mediated through dopamine and serotonin type 2 antagonism
Dosage
- Initial dose is 5-10 mg once daily for schizophrenia and bipolar disorder
- Maximum dose is 20 mg daily
Administration
- Can be given without regard to meals
- Gradual tapering of the dose should be considered while discontinuing
Interaction
- May antagonize the effects of levodopa and dopamine agonists
- Influenced by CYP1A2 inducers and inhibitors
- Activated charcoal reduces oral bioavailability by 50-60%
Contraindications
- Hypersensitivity to any ingredient
- Known risk for narrow-angle glaucoma
Side Effects
- Somnolence and weight gain
- Elevated glucose and triglyceride levels
- Dizziness, akathisia, Parkinson's disease
- Dry mouth, asthenia, edema, photosensitivity reaction
Pregnancy & Lactation
- Use in pregnancy only if potential benefits justify the potential risk to the fetus
- No report to show teratogenicity
- Not recommended during breastfeeding
Precautions & Warnings
- Use cautiously in patients with history of seizures or conditions associated with seizures
- Cautious use in patients with low leukocyte and/or neutrophil counts
- Caution about operating hazardous machinery
Use in Special Populations
- Not studied in subjects under 18 years of age
- Starting dose for elderly patients (age 65 and over) and patients with hepatic or renal impairment
