Indications

  • Treatment of schizophrenia
  • Management of bipolar disorder
  • Treatment of acute manic or mixed episodes in bipolar disorder

Pharmacology

  • Affinities for serotonin and dopamine receptors
  • Mechanism of action is unknown
  • Well absorbed after oral administration

Dosage

  • Initial dose is 5-10 mg once daily
  • Maximum dose is 20 mg daily
  • Different dosages for combination therapy and monotherapy for mania

Administration

  • Can be given without regard to meals
  • Gradual tapering of the dose should be considered while discontinuing

Interaction

  • May antagonize the effects of levodopa and dopamine agonists
  • Inhibitors of CYP1A2 may potentially inhibit Lopez elimination
  • Activated charcoal reduces the oral bioavailability of Lopez by 50-60%

Contraindications

  • Known hypersensitivity to any ingredient of the product
  • Risk for narrow-angle glaucoma

Side Effects

  • Common undesirable effects include somnolence and weight gain
  • Other effects include increased appetite, elevated glucose levels, elevated triglyceride levels, dizziness, akathisia, and more

Pregnancy & Lactation

  • Should be used in pregnancy only if the potential benefits justify the potential risk to the fetus
  • Patients should not breastfeed while taking Olanzapine

Precautions & Warnings

  • Should be used cautiously in patients with a history of seizures
  • Cautions related to low leukocyte and/or neutrophil counts, impaired hepatic function, and increased QT interval
  • Patients should be cautioned about operating hazardous machinery, including motor vehicles

Use in Special Populations

  • Not studied in subjects under 18 years of age
  • Different starting doses for elderly patients, patients with hepatic or renal impairment
