Dosage

  • Initial dose 5-10 mg once daily for schizophrenia and bipolar disorder
  • Daily dosage may be adjusted within the range of 5-20 mg daily
  • Maximum dose is 20 mg daily
  • Initial dose 15 mg once daily for monotherapy for mania
  • Maximum dose for monotherapy is 20 mg daily

Administration

  • Can be given without regard to meals
  • Gradual tapering of the dose should be considered while discontinuing

Interaction

  • Antagonizes effects of levodopa and dopamine agonists
  • Influenced by drugs that induce CYP1A2 or glucoronyl transferase enzymes
  • Inhibitors of CYP1A2 may potentially inhibit elimination
  • Carbamazepine may increase clearance
  • Activated charcoal reduces oral bioavailability by 50-60%
  • Caution required with centrally acting drugs and alcohol

Contraindications

  • Known hypersensitivity to any ingredient
  • Known risk for narrow-angle glaucoma

Precautions & Warnings

  • Cautious use in patients with history of seizures
  • Cautious use in patients with low leukocyte and/or neutrophil counts
  • Cautious use in patients with impaired hepatic function
  • Cautious use in patients using hepatotoxic medicines
  • Cautious use in patients using centrally acting drugs
  • Cautious use in patients using medicines known to increase QT interval
  • Cautious use in the elderly
  • Patients cautioned about operating hazardous machinery

Use in Special Populations

  • Not studied in subjects under 18 years of age
  • Starting dose of 5 mg/day for elderly patients
  • Starting dose of 5 mg/day for patients with hepatic or renal impairment
