Dosage
- Initial dose 5-10 mg once daily for schizophrenia and bipolar disorder
- Daily dosage may be adjusted within the range of 5-20 mg daily
- Maximum dose is 20 mg daily
- Initial dose 15 mg once daily for monotherapy for mania
- Maximum dose for monotherapy is 20 mg daily
Administration
- Can be given without regard to meals
- Gradual tapering of the dose should be considered while discontinuing
Interaction
- Antagonizes effects of levodopa and dopamine agonists
- Influenced by drugs that induce CYP1A2 or glucoronyl transferase enzymes
- Inhibitors of CYP1A2 may potentially inhibit elimination
- Carbamazepine may increase clearance
- Activated charcoal reduces oral bioavailability by 50-60%
- Caution required with centrally acting drugs and alcohol
Contraindications
- Known hypersensitivity to any ingredient
- Known risk for narrow-angle glaucoma

- Cautious use in patients with history of seizures
- Cautious use in patients with low leukocyte and/or neutrophil counts

- Cautious use in patients using medicines known to increase QT interval
