Indications
- Acute and maintenance treatment of schizophrenia
- Treatment of acute manic or mixed episodes in bipolar disorder
Pharmacology
- Antipsychotic agent with affinities for serotonin, dopamine, cholinergic muscarinic receptors, adrenergic, and histamine receptors
- Well absorbed after oral administration, reaching peak plasma concentrations within 5 to 8 hours
Dosage
- Initial dose of 5-10 mg once daily for schizophrenia and combination therapy for mania
- Max daily dosage of 20 mg
- Initial dose of 15 mg once daily for monotherapy for mania
Administration
- Can be given without regard to meals
- Gradual tapering of the dose should be considered while discontinuing
Interaction
- May antagonize the effects of levodopa and dopamine agonists
- Inhibitors of CYP1A2 may potentially inhibit elimination
- Activated charcoal reduces oral bioavailability by 50-60%
Contraindications
- Known hypersensitivity to any ingredient
- Known risk for narrow-angle glaucoma
Side Effects
- Very common undesirable effects include somnolence and weight gain
- Other effects like increased appetite, dizziness, elevated glucose and triglyceride levels, etc. may be observed
Pregnancy & Lactation
- Should be used in pregnancy only if potential benefits justify the potential risk to the fetus
- Patients should not breastfeed if taking Olanzapine
Precautions & Warnings
- Use cautiously in patients with a history of seizures
- Use cautiously in patients with low leukocyte and/or neutrophil counts
- Use cautiously in patients with impaired hepatic function
- Patients should be cautioned about operating hazardous machinery
Use in Special Populations
- Not studied in subjects under 18 years of age
- Starting dose for elderly patients (age 65 and over) is 5 mg/day
- Starting dose for patients with hepatic or renal impairment is 5 mg/day
