Indications
- Treatment of acute and chronic psychoses

Pharmacology

Dosage & Administration
- Twice-daily administration for acute and chronic psychoses and schizophrenia
- Once or twice-daily administration for bipolar disorder
- Total daily dose range of 150 to 750 mg for acute and chronic psychoses and schizophrenia
- Usual effective dose range of 300-450 mg/day for acute and chronic psychoses and schizophrenia
- Total daily dose range of 200 to 800 mg for manic episodes associated with bipolar disorder
- Usual effective dose range of 400 to 800 mg/day for manic episodes associated with bipolar disorder
- Usual dose of 300 mg/day for depressive episodes associated with bipolar disorder
- Usual dose range of 400 to 600 mg/day for depressive episodes associated with bipolar disorder
- Usual dose range of 400 to 800 mg/day for maintenance treatment of bipolar I disorder
Interaction
- Potential interaction with medicines causing electrolyte imbalance
- Potential interaction with medicines increasing QT interval
- Potential interaction with thioridazine, carbamazepine, and phenytoin
Contraindications
- Contra-indicated in patients hypersensitive to it
Side Effects
- Common adverse drug reactions include somnolence, dizziness, dry mouth, and weight gain
Pregnancy & Lactation
- Safety and efficacy during human pregnancy not established
- Use only if benefits justify potential risks
- Avoid breastfeeding while taking the medication
Precautions & Warnings
- Increased risk of suicidal thoughts and self-harm in patients with depression
- Use with caution in patients with cardiovascular or cerebrovascular disease
- Caution in patients with history of seizures
- Consider dose reduction or discontinuation if signs of tardive dyskinesia appear
- Discontinue in case of neuroleptic malignant syndrome
- Exercise caution in patients with a history of QT prolongation or cardiovascular disease
- Monitor for neutropenia and hyperglycemia
Use in Special Populations
- Use with caution in the elderly
- Not indicated for use in children and adolescents below 18 years of age
Overdose Effects
- Survival reported in acute overdoses of up to 30 grams
- No specific antidote available
- Intensive care procedures recommended
- Treat refractory hypotension with appropriate measures
