Indications
- Treatment of acute and chronic psychoses
- Schizophrenia treatment
- Bipolar disorder treatment
- Maintenance treatment of bipolar I disorder
- Prevention of recurrence of manic and depressive episodes
Pharmacology
- Belongs to dibenzothiazepine derivatives
- Antagonist at multiple neurotransmitter receptors
- No appreciable affinity at cholinergic muscarinic and benzodiazepine receptors
- Mechanism of action is unknown
- Efficacy in schizophrenia may be mediated through dopamine D2 and serotonin 5HT2 antagonism
Dosage & Administration
- Administered twice daily
- Total daily dose titration range from 150 to 750 mg/day
- Usual effective dose range of 300-450 mg/day for schizophrenia
- Usual effective dose range of 400-800 mg/day for manic episodes
- Usual dose for depressive episodes is 300 mg/day
Interaction
- Caution with medicines known to cause electrolyte imbalance
- Co-administration with thioridazine or carbamazepine increases clearance of Tiapine
- Co-administration with phenytoin increases clearance of Tiapine
Contraindications
- Hypersensitivity to Quetiapine
Side Effects
- Somnolence
- Dizziness
- Dry mouth
- Elevations in serum triglyceride levels
- Weight gain
- Extrapyramidal symptoms
Pregnancy & Lactation
- Safety and efficacy not established during human pregnancy
- Use only if benefits outweigh potential risks
- Avoid breastfeeding while taking Quetiapine
Precautions & Warnings
- Increased risk of suicidal thoughts and self-harm
- Caution in patients with cardiovascular or cerebrovascular disease
- Recommended caution in treating patients with a history of seizures
- Consider dose reduction or discontinuation if tardive dyskinesia appears
- Discontinue if neuroleptic malignant syndrome is suspected
- Exercise caution in patients with family history of QT prolongation
- Severe neutropenia reported in clinical trials
- Increases in blood glucose and hyperglycemia observed
Use in Special Populations
- Caution in the elderly, slower dose titration and lower daily therapeutic dose may be needed
- Not indicated for use in children and adolescents below 18 years of age
