Indications

  • Treatment of Acute and chronic psychoses
  • Treatment of schizophrenia
  • Treatment of Bipolar Disorder
  • Maintenance treatment of bipolar I disorder
  • Combination therapy with a mood stabilizer
  • Treatment of manic episodes satisfying DSM-IV criteria
  • Treatment of depressive episodes associated with bipolar disorder
  • Prevention of recurrence of manic, depressive or mixed episodes

Pharmacology

  • Belongs to a chemical class, the dibenzothiazepine derivatives
  • Antagonist at multiple neurotransmitter receptors in the brain
  • Antagonist at serotonin 5HT1A and 5HT2 receptors
  • Antagonist at dopamine D1 and D2 receptors
  • Antagonist at histamine H1 receptor
  • Antagonist at adrenergic α1 and α2 receptors
  • No appreciable affinity at cholinergic muscarinic and benzodiazepine receptors

Interaction

  • Hypersensitivity to Quetiapine

  • Elevations in serum triglyceride levels
  • Elevations in total cholesterol
  • Decreases in HDL cholesterol
  • Weight gain
  • Decreased haemoglobin
  • Extrapyramidal symptoms

Pregnancy & Lactation

  • Safety and efficacy during human pregnancy not established
  • Use during pregnancy if benefits justify potential risks
  • Lowest possible dose and duration of treatment during pregnancy
  • Excretion into human milk is unknown
  • Avoid breastfeeding while taking Quetiapine

Precautions & Warnings

  • Risk of suicidal thoughts and self-harm
  • Caution in patients with known cardiovascular disease
  • Caution in patients with cerebrovascular disease
  • Caution in patients predisposed to hypotension
  • Caution in patients with history of seizures
  • Consideration of dose reduction or discontinuation for tardive dyskinesia
  • Discontinuation and appropriate medical treatment for Neuroleptic Malignant Syndrome
  • Caution in patients with cardiovascular disease or family history of QT prolongation
  • Reports of severe neutropenia
  • Observation of increases in blood glucose and hyperglycemia

Use in Special Populations

  • Caution in the elderly
  • Slower rate of dose titration in the elderly
  • Lower daily therapeutic dose in the elderly
  • Not indicated for use in children and adolescents below 18 years of age

Overdose Effects

  • Reported survival in acute overdoses of up to 30 grams of Seroquet
  • No specific antidote to Seroquet
  • Consideration of multiple drug involvement in severe intoxication
  • Intensive care procedures recommended
  • Treatment of refractory hypotension with appropriate measures
  • Close medical supervision and monitoring
