Indications
- Treatment of acute and chronic psychoses
- Treatment of schizophrenia
- Treatment of bipolar disorder including manic and depressive episodes
- Maintenance treatment of bipolar I disorder
- Prevention of recurrence of manic, depressive, or mixed episodes
Pharmacology
- Atypical psychotropic agent
- Antagonist at multiple neurotransmitter receptors
- Mechanism of action is unknown
- Efficacy in schizophrenia mediated through dopamine D2 and serotonin 5HT2 antagonism
- Antagonism of histamine H1 receptors may cause somnolence
- Antagonism of adrenergic a receptors may cause orthostatic hypotension

Interaction
- Caution with medicines causing electrolyte imbalance

Side Effects

Precautions & Warnings
- Increased risk of suicidal thoughts and self-harm
- Caution in patients with known cardiovascular disease
- Caution in patients with history of seizures
- Consideration of dose reduction or discontinuation if tardive dyskinesia symptoms appear
- Caution in patients with family history of QT prolongation
- Uncommon reports of severe neutropenia, hyperglycemia, and diabetes mellitus
Use in Special Populations

Overdose Effects
- Survival reported in acute overdoses of up to 30 grams
