Indications
- Treatment of acute and chronic psychoses
- Treatment of mania
- Treatment of schizophrenia
Pharmacology
- Selective monoaminergic antagonist
- High affinity for serotoninergic 5-HT2 and dopaminergic D2 receptors
- Binds also to alpha1 adrenergic receptors
- Binds with lower affinity to H1 histamine and alpha2 adrenergic receptors
- No affinity for cholinergic receptors
- Reduces extrapyramidal side effect liability
- Improves positive symptoms of schizophrenia
- Balanced central serotonin and dopamine antagonism
Dosage & Administration
- Initial dose of 2 mg in 1-2 divided doses on the first day
- Usual dose range of 4-6 mg daily
- Max dose of 16 mg daily
- Lower initial dose for elderly patients or those with hepatic or renal impairment
- Initial dose of 1 mg daily for elderly patients or those with hepatic or renal impairment
- Dose adjustments for mania treatment
- Dose adjustments for schizophrenia treatment
- Dose adjustments at intervals of not less than 1 week
- Small dose increments/decrements of 1-2 mg recommended for dosage adjustments
Interaction
- Antagonizes the effects of levodopa and dopamine agonists
- Reduces plasma clearance with chronic administration of Carbamazepine
- May decrease the clearance with chronic administration of Clozapine
- Enhances the effects of certain antihypertensives
