Indications
- Treatment of insomnia characterized by difficulty with sleep maintenance
Pharmacology
- Binds with high affinity to the histamine H1 receptor
- Functions as an antagonist of the H1 receptor
Dosage & Administration
- Recommended dose for adults is 6 mg once daily
- Starting dose in elderly patients is 3 mg once daily
- To be taken within 30 minutes of bedtime
- Total dose should not exceed 6 mg per day
Interaction
- Primarily metabolized by hepatic cytochrome P450 isozymes CYP2C19 and CYP2D6
- Exposure doubled with concomitant administration of cimetidine
- Sedative effects of alcohol and CNS depressants may be potentiated
Contraindications
- Contraindicated in individuals with hypersensitivity to doxepin HCl or its inactive ingredients
- Do not administer if patient is currently on MAOIs
- Contraindicated in individuals with untreated narrow-angle glaucoma or severe urinary retention
Side Effects
- Abnormal thinking and behavioral changes
- Suicide risk and worsening of depression
- CNS depressant effects
Pregnancy & Lactation
- Pregnancy Category C
- Excreted in human milk after oral administration
Precautions & Warnings
- Initiate treatment only after careful evaluation of the patient
- Worsening of depression and increased suicide risk reported in association with use of hypnotics
- Discontinuation should be strongly considered for patients who report a 'sleep-driving' episode
