Description
Original tricyclic antidepressant, member of the dibenzazepine group of compounds.
Indications
- Depressive illness
- Nocturnal enuresis in children
Dosage & Administration
- Initial dose: up to 75 mg daily
- Maximum dose: 150-200 mg (up to 300 mg in hospital patients)
- Child dosage for nocturnal enuresis: 25-75 mg at bedtime
Pharmacology
Inhibits neuronal reuptake of norepinephrine and serotonin, contributing to changes in neurotransmission and brain physiology to relieve symptoms of depression.
Interaction
- May increase effects of methylphenidate
- May increase depressant action of alcohol
- Should not be taken with MAO inhibitors
- Anticholinergic effects are additive with other drugs
Contraindications
- Should not be taken with or within 14 days of MAO inhibitors
- Contraindicated in severe hepatic or renal damage
- Hypersensitivity to the drug or tricyclic antidepressants
- Acute recovery phase following myocardial infarction
- Patients with convulsive disorders or glaucoma

Pregnancy & Lactation
Pregnancy category D, excreted in human breast milk, known risk of damage to fetus
Precautions & Warnings
- Use cautiously in elderly with benign prostatic hypertrophy, urinary retention, and glaucoma
- Increased sedative effect with other CNS depressants
- May increase heart rate and stress on the heart
- Close monitoring for signs of suicide
- Increased risk of suicide with overdose or alcohol combination
- Manic episodes and emergence of psychotic states reported during therapy
Overdose Effects
Children may be more sensitive than adults, acute overdose in infants or young children can be serious and potentially fatal
