Description
- Tricyclic antidepressant
- Dibenzazepine compound
- Mechanism of action not fully known
- Potentiates adrenergic synapses
- Blocks uptake of norepinephrine
Pharmacology
- Inhibits neuronal reuptake of norepinephrine and serotonin
- Binds sodium-dependent serotonin transporter
- Reduces reuptake of neurotransmitters
- Linked to changes in neurotransmission and brain physiology
Dosage & Administration
- Initial dosage up to 75 mg daily
- Increased gradually to 150-200 mg
- Up to 300 mg in hospital patients
- Up to 150 mg may be given as a single bedtime dose
- Elderly initial dosage 10 mg daily
- Child not recommended for depression
- Children's dosage for nocturnal enuresis by age
Interaction
- Methylphenidate may increase effects
- May increase depressant action of alcohol
- Dangerously high blood pressure with MAO inhibitors
- Anticholinergic effects are additive with other drugs
Contraindications
- Do not use with MAO inhibitors
- Contraindicated in severe hepatic or renal damage
- Contraindicated in history of blood dyscrasias
- Hypersensitivity to the drug or tricyclic antidepressants
- Not for use during acute recovery phase after myocardial infarction
- Not for use in convulsive disorders or glaucoma

Precautions & Warnings

- Risk for cardiovascular disease patients
- Need for close monitoring during initial dosage
- Risk of suicide especially in overdose or with alcohol
- Emergence of pre-existing psychotic states

- Children more sensitive than adults
