Indications

• Treatment of depressive illness
• Relief of anxiety associated with depression

Pharmacology

• Mechanism of action is not precisely known
• Acts primarily by potentiation of central adrenergic synapses
• Responsible for antidepressant and anxiolytic effects

Dosage & Administration

• Single daily dose as an alternative to divided daily doses
• Therapeutic effects may be seen within 3 to 7 days
• Initial Adult Dosage: Suggested initial dosage of 75 mg daily for outpatients with mild to moderate depression
• Elderly Patients: Lower dosages are recommended for patients over 60 years of age

Interaction

• Close supervision and careful dosage adjustment required when administering with anticholinergic or sympathomimetic drugs
• Concurrent administration with electroshock therapy should be avoided
• Caution when administering to hyperthyroid patients or those on thyroid medication

Contraindications

• Contraindicated in patients hypersensitive to maprotiline
• Not to be given concomitantly with monoamine oxidase (MAO) inhibitors

Side Effects

• Cardiovascular: Rare occurrences of hypotension, hypertension, tachycardia
• Psychiatric: Nervousness, anxiety, insomnia, agitation, confusional states
• Neurological: Drowsiness, dizziness, tremor, numbness, tingling
• Anticholinergic: Dry mouth, constipation, blurred vision
• Allergic: Skin rash, itching, photosensitization
• Gastrointestinal: Nausea, vomiting, abdominal cramps
• Endocrine: Increased or decreased libido, impotence, alteration of blood sugar levels

Pregnancy & Lactation

• Pregnancy Category B
• Reproduction studies performed in female laboratory animals
• Drug excreted in breast milk

Precautions & Warnings

• Careful supervision of seriously depressed patients
• Possibility of hypomanic or manic episodes
• Caution in patients with a history of urinary retention or narrow-angle glaucoma
