Indications
- Treatment of acute headache phase of migraine attacks
- Treating migraine with or without aura in adolescents aged 12 to 17 years
Pharmacology
- Selective and potent serotonin (5-hydroxytryptamine 1B/1D) agonist
- Binds to specific serotonin receptors on meningeal arteries
- Inhibits release of vasoactive peptides
- Causes constriction of the arteries
- Limited effect on arteries supplying blood to the brain
- Little effect on cardiac and pulmonary vessels
Dosage & Administration
- Recommended dose in adults and adolescents: 6.25 mg to 12.5 mg
- 12.5 mg dose tends to be more effective in adults
- Maximum daily dose not to exceed 25 mg
- Recommended starting dose in hepatic impairment patients: 6.25 mg
- Maximum daily dose in hepatic impairment: 12.5 mg
- Recommended starting dose in severe renal impairment patients: 6.25 mg
- Maximum daily dose in severe renal impairment: 12.5 mg
Interaction
- May cause prolonged vasospastic reactions
- Risk of serotonin syndrome with SSRIs or SNRIs
Contraindications
- Not to be used in patients with history, symptoms, or signs of ischaemic heart disease
- Contraindicated with ergotamine, ergotamine derivatives, and other 5-HT1B/1D agonists
Side Effects
- Serious cardiac reactions reported, including myocardial infarction
Pregnancy & Lactation
- Pregnancy Category C
- No data on excretion of almotriptan in human milk

- Hypersensitivity to the active substance or excipients
- Severe hepatic impairment
- Previous cerebrovascular accident (CVA) or transient ischaemic attack (TIA)
- Peripheral vascular disease
