Indications
- Treatment of major depressive episodes
- Treatment of panic disorder with or without agoraphobia
- Treatment of social anxiety disorder (social phobia)
- Treatment of generalised anxiety disorder
- Treatment of obsessive-compulsive disorder
Pharmacology
- Selective serotonin reuptake inhibitor (SSRI)
- Pure S-enantiomer of the racemic bicyclic phthalate derivative citalopram
- 100 fold more potent than the R-enantiomer with respect to inhibition of 5-HT reuptake
- Low affinity for serotonergic or other receptors including alpha- and beta-adrenergic dopamine, histamine, muscarinic and benzodiazepine receptors
Dosage
- Administration as a single daily dose
- Safe daily dose up to 20 mg
- Usual dosage of 10 mg once daily for major depressive episodes
- Initial dose of 5 mg for panic disorder with or without agoraphobia
- Social anxiety disorder: 10 mg once daily, with possible increase to 20 mg daily
- Generalised anxiety disorder: Initial dosage is 10 mg once daily, with possible increase to 20 mg daily
- Obsessive-Compulsive Disorder: Initial dosage is 10 mg once daily, with possible increase to 20 mg daily
Administration
- Administered once daily, morning or evening
- May be taken with or without food
Interaction
- SSRI or related antidepressants should not be started until 2 weeks after stopping an MAOI
- An MAOI should not be started until at least a week after stopping an SSRI or related antidepressant
Contraindications
- Hypersensitivity to Escitalopram or Citalopram or any of the inactive ingredients
- Concomitant use of escitalopram in patients taking monoamine oxidase/pimozide
Side Effects
- Less sedating and have fewer antimuscarinic and cardiotoxic effects than tricyclic antidepressants
- Gastrointestinal effects, anorexia with weight loss or increased appetite and weight gain, hypersensitivity reactions, dry mouth, nervousness, etc.
Pregnancy & Lactation
- Potential risks and benefits of treatment for pregnant women
- Excretion in human breast milk
Precautions & Warnings
- Use with caution in patients with epilepsy, cardiac disease, diabetes mellitus, angle-closure glaucoma, etc.
Use in Special Populations
- Elderly patients (>65 years of age): Initial dosage is 5 mg once daily, with possible increase to 10 mg daily
- Children and adolescents (<18 years): Not recommended
- Dosage adjustment in reduced renal or hepatic function
- Caution in poor metabolizers of CYP2C19
- Gradual reduction of dose when stopping treatment
Overdose Effects
- Symptoms mainly related to the central nervous system, gastrointestinal system, cardiovascular system, and electrolyte/fluid balance conditions
- No specific antidote
- Establish and maintain an airway, gastric lavage and activated charcoal may be considered, ECG monitoring advised
