Description

  • Indications:
    • Treatment of major depressive episodes
    • Treatment of panic disorder with or without agoraphobia
    • Treatment of social anxiety disorder (social phobia)
    • Treatment of generalised anxiety disorder
    • Treatment of obsessive-compulsive disorder
  • Pharmacology: Escitalopram is an orally administered SSRI with potent inhibition of 5-HT reuptake
  • Dosage:
    • Safe daily dose: Not demonstrated above 20 mg
    • Usual dosage for major depressive episodes: 10-20 mg once daily
    • Initial dose for panic disorder: 5 mg then increase to 10-20 mg daily
    • Initial dose for social anxiety disorder: 10 mg, may increase to 20 mg daily
    • Initial dose for generalised anxiety disorder: 10-20 mg once daily
    • Initial dose for obsessive-compulsive disorder: 10-20 mg once daily
  • Administration: Administered once daily, morning or evening, with or without food
  • Interaction: Avoid starting MAOI until 2 weeks after stopping SSRI, and vice versa
  • Contraindications:
    • Hypersensitivity to Escitalopram or Citalopram
    • Concomitant use with MAOI/pimozide
  • Side Effects:
    • Less sedating than tricyclic antidepressants
    • Common side effects include gastrointestinal effects, anorexia, weight changes, hypersensitivity reactions, dry mouth, nervousness, headache, insomnia, tremor, dizziness, sexual dysfunction, and more
  • Pregnancy & Lactation: Physician should carefully consider risks and benefits of treatment during the third trimester, excreted in breast milk
  • Precautions & Warnings: Caution in patients with epilepsy, cardiac disease, diabetes, glaucoma, bleeding disorders, hepatic and renal impairment
  • Use in Special Populations:
    • Dosage adjustment in elderly patients
    • Not recommended for children and adolescents
    • Caution in patients with renal, hepatic impairment, and poor metabolizers of CYP2C19
  • Overdose Effects:
    • Symptoms include central nervous system, gastrointestinal, cardiovascular, and electrolyte/fluid balance conditions
    • Management involves maintaining airway and oxygenation, gastric lavage, activated charcoal, ECG monitoring
  • Therapeutic Class: SSRIs & related anti-depressant drugs
  • Storage Conditions: Store below 30°C, protect from light & moisture, keep out of reach of children
