Indications
- Treatment of major depressive episodes
- Treatment of panic disorder with or without agoraphobia
- Treatment of social anxiety disorder (social phobia)
- Treatment of generalised anxiety disorder
- Treatment of obsessive-compulsive disorder
Pharmacology
- Orally administered
- Selective serotonin reuptake inhibitor (SSRI)
- Pure S-enantiomer of racemic bicyclic phthalate derivative citalopram
- At least 100 fold more potent than the R-enantiomer
- No or very low affinity for serotonergic or other receptors including alpha- and beta-adrenergic Dopamine, Histamine, Muscarinic and benzodiazepine receptors
Dosage
- Maximum dosage of 20 mg daily
- Usual dosage of 10 mg once daily for major depressive episodes
- Initial dose of 5 mg recommended for panic disorder with or without agoraphobia
- Usual dosage of 10 mg once daily for social anxiety disorder
- Initial dosage of 10 mg once daily for generalised anxiety disorder
- Initial dosage of 10 mg once daily for Obsessive-Compulsive Disorder
Administration
- Generally administered once daily
- Can be taken in the morning or evening
- Can be taken with or without food
Interaction
- Not to be started until 2 weeks after stopping a MAOI
- Should not be started until at least a week after stopping an SSRI or related antidepressant
- Specific periods for certain antidepressants
Contraindications
- Contraindicated in patients with known hypersensitivity to Escitalopram or Citalopram
- Contraindicated in concomitant use with monoamine oxidase/pimozide
Side Effects
- Less sedating and have fewer antimuscarinic and cardiotoxic effects than tricyclic antidepressants
- Common side effects include nausea, vomiting, dyspepsia, abdominal pain, diarrhoea, constipation
- Other side effects include anorexia with weight loss, hypersensitivity reactions, dry mouth, nervousness, anxiety, headache, insomnia, tremor, dizziness, sexual dysfunction
Pregnancy & Lactation
- Consider risks and benefits when treating pregnant women during the third trimester
- Excreted in human breast milk, consideration for nursing
Precautions & Warnings
- Use with caution in patients with epilepsy, cardiac disease, diabetes mellitus, angle-closure glaucoma
- Caution in patients with hepatic and renal impairment
- Avoid in poorly controlled epilepsy, discontinue if convulsions develop
Use in Special Populations
- Initial dosage of 5 mg once daily for elderly patients (>65 years of age)
- Not to be used in children and adolescents (<18 years)
- Dosage adjustment not necessary in patients with mild or moderate renal impairment
- Caution in patients with severely reduced renal function, hepatic function
- Consideration for poor metabolizers of CYP2C19
Overdose Effects
- Symptoms include dizziness, tremor, agitation, serotonin syndrome, convulsion, coma, nausea/vomiting, hypotension, arrhythmia, electrolyte/fluid balance conditions
- No specific antidote, general symptomatic supportive measures recommended
