Pharmacology
Serotonin and norepinephrine reuptake inhibitor (SNRI) for oral administration. Less potent inhibitor of dopamine reuptake. No significant affinity for various receptors in vitro. Well absorbed. Mainly eliminated through hepatic metabolism.
Dosage & Administration
- Major Depressive Disorder (MDD):
- Starting Dose:
- 40 mg/day to 60 mg/day
- Target Dose:
- Acute: 40 mg/day (20 mg twice daily) to 60 mg/day (once daily or as 30 mg twice daily)
- Maintenance: 60 mg/day

Interaction
Metabolism affected by CYP1A2 and CYP2D6 isozymes. Co-administration with inhibitors may increase drug concentrations.
Contraindications
- Known hypersensitivity to the drug or its inactive ingredients
- Not approved for treating bipolar depression
- Not for patients with substantial alcohol use or chronic liver disease
- Use cautiously in patients with controlled narrow-angle glaucoma

Pregnancy & Lactation
- Pregnancy: Category C. Use only if potential benefit justifies potential risk to the fetus.
- Labor and Delivery: Effects unknown. Use only if potential benefit justifies potential risk to the fetus.
- Lactation: Not recommended
Precautions & Warnings
- Monitor patients for clinical worsening, suicidality, and unusual behavior
- Measure blood pressure prior to treatment and periodically
- Caution about risk of bleeding with concomitant use of Duzela and NSAIDs/other coagulation-affecting drugs
- Use cautiously in patients with history of mania/seizure disorder
Use in Special Populations
- Safety and efficacy in pediatric patients not established
Overdose Effects
Limited clinical experience. No specific antidote. General overdose management measures may be employed.
