Indications

  • Major Depressive Disorder (MDD)

Pharmacology

  • Selective serotonin and norepinephrine reuptake inhibitor (SSNRI)
  • Less potent inhibitor of dopamine reuptake
  • No significant affinity for various receptors
  • Mainly eliminated through hepatic metabolism

Dosage & Administration

  • Starting Dose: 40 mg/day to 60 mg/day for MDD
  • Target Dose: 60 mg/day for GAD
  • Maximum Dose: 120 mg/day for MDD and GAD
  • Starting Dose: 60 mg/day for DPNP
  • Target Dose: 60 mg/day for DPNP
  • Maximum Dose: 60 mg/day for DPNP
  • Starting Dose: 30 mg/day for Fibromyalgia
  • Target Dose: 60 mg/day for Fibromyalgia
  • Maximum Dose: 60 mg/day for Fibromyalgia
  • Starting Dose: 30 mg/day for Chronic Musculoskeletal Pain
  • Target Dose: 60 mg/day for Chronic Musculoskeletal Pain
  • Maximum Dose: 60 mg/day for Chronic Musculoskeletal Pain
  • No evidence for benefit above 60 mg/day
  • Gradual dose reduction recommended
  • Avoid concomitant use with certain inhibitors

Interaction

  • Metabolism via CYP1A2 and CYP2D6 isozymes
  • Increased levels when co-administered with specific inhibitors

Contraindications

  • Hypersensitivity to drug or inactive ingredients
  • Bipolar depression
  • Substantial alcohol use or chronic liver disease
  • Use caution in patients with narrow-angle glaucoma

  • Constipation
  • Decreased appetite
  • Fatigue
  • Somnolence
  • Increased sweating
  • Hyperhidrosis
  • Asthenia
  • Slight increase in blood pressure
  • No clinically significant differences in cardiac intervals

Pregnancy & Lactation

  • Pregnancy Category C
  • Use during pregnancy only if benefit justifies risk
  • Effect on labor and delivery unknown
  • Not recommended during lactation

Precautions & Warnings

  • Monitoring for clinical worsening, suicidality, and behavioral changes
  • Monitoring blood pressure
  • Caution with concomitant use of NSAIDs, aspirin, or coagulation-affecting drugs
  • Caution in patients with history of mania or seizure disorder

Use in Special Populations

  • Safety and efficacy not established in pediatric patients

Overdose Effects

  • Limited clinical experience with overdose
  • No specific antidote
  • General measures for overdose management
  • Monitoring airway, oxygenation, ventilation, cardiac rhythm and vital signs
  • Avoid induction of emesis
  • Consider gastric lavage and activated charcoal
