Indications

  • Major Depressive Disorder (MDD)
  • Generalized Anxiety Disorder (GAD)
  • Diabetic Peripheral Neuropathic Pain (DPNP)
  • Fibromyalgia
  • Chronic Musculoskeletal Pain

Pharmacology

  • Selective serotonin and norepinephrine reuptake inhibitor (SSNRI)
  • Less potent inhibitor of dopamine reuptake
  • Well absorbed through oral administration
  • Metabolism mainly through hepatic metabolism

Dosage & Administration

  • Starting Dose: 30 mg/day to 60 mg/day
  • Target Dose: 40 mg/day to 60 mg/day
  • Maximum Dose: 120 mg/day
  • Some patients may benefit from starting at 30 mg once daily
  • Gradual dose reduction recommended to avoid discontinuation symptoms

Interaction

  • Metabolism influenced by CYP1A2 and CYP2D6 isozymes
  • Potent inhibitors of CYP1A2 and CYP2D6 may increase drug concentrations
  • Avoid co-administration with fluvoxamine, cimetidine, quinolone antimicrobials, and potent inhibitors of CYP2D6

Contraindications

  • Known hypersensitivity to the drug or any inactive ingredients
  • Not approved for treating bipolar depression
  • Patients with substantial alcohol use or chronic liver disease
  • Cautious use in patients with controlled narrow-angle glaucoma

Precautions & Warnings

  • Close monitoring for clinical worsening and unusual changes in behavior
  • Blood pressure measurement prior to treatment and periodically throughout
  • Caution about the risk of bleeding when used with NSAIDs, aspirin, or other coagulation-affecting drugs
  • Careful use in patients with a history of mania or seizure disorder

Use in Special Populations

  • Safety and efficacy in pediatric patients not established

Overdose Effects

  • Limited clinical experience with overdose in humans
