Description
- Fluoxetine is indicated for the treatment of depressive illness, bulimia nervosa, and obsessive-compulsive disorder. It selectively inhibits the reuptake of serotonin, leading to enhanced serotonergic neurotransmission. The initial recommended dose is 20 mg/day, with a maximum dose of 80 mg/day. It is contraindicated in patients known to be hypersensitive to it and should not be used in combination with MAOIs.

Pharmacology
- Selective inhibition of serotonin reuptake at the presynaptic neuronal membrane
- Increased synaptic concentration of serotonin in the CNS
- Functional changes associated with enhanced serotonergic neurotransmission
- No effect on the reuptake of norepinephrine and dopamine
- No anticholinergic, antihistaminic or α1 adrenergic blocking activity at usual therapeutic doses
Dosage & Administration
- Initial treatment: 20 mg/day administered in the morning
- Consider dose increase after several weeks if no clinical improvement is observed
- Maintenance treatment: several months or longer sustained pharmacologic therapy
- Children: Not recommended
- Lower or less frequent dosage for patients with renal and/or hepatic impairment or elderly patients

Contraindications
- Hypersensitivity to Fluoxetine
- Concomitant use with MAOIs or within 14 days of discontinuing therapy with MAOI
- At least 5 weeks should be allowed after stopping Fluoxetine before starting MAOI
Side Effects
- Gastrointestinal - Nausea, vomiting, dyspepsia, dry mouth, diarrhea
- Neurological - Anxiety, nervousness, insomnia/drowsiness, fatigue
- Others - Excessive sweating, pruritus, skin rashes associated with liver, kidney and lung involvement
Pregnancy & Lactation
- Use in pregnancy only if clearly needed
- Caution in nursing women due to excretion in human milk
Precautions & Warnings
- Use with caution and in reduced doses in patients with impaired hepatic or renal function
- Caution in patients with epilepsy or a history of such disorders
- Caution warranted in diabetic subjects
- Careful supervision of depressed patients with suicidal tendencies
- Not usually considered suitable for the depressive component of bipolar (manic depressive) illness
