Indications
- depressive illness
- bulimia nervosa
- obsessive compulsive disorder
Pharmacology
- selective inhibition of serotonin reuptake
- increased synaptic concentration of serotonin
- functional changes associated with enhanced serotonergic neurotransmission
Dosage & Administration
- initial dose of 20 mg/day
- dose increase after several weeks if no improvement
- maximum dose of 80 mg/day
- full antidepressant effect may be delayed until 4 weeks of treatment
- lower or less frequent dosage for patients with renal and/or hepatic impairment
- maintenance treatment for several months or longer
- not recommended for use in children
Interaction
- serious fatal reactions with MAO inhibitors
- increase of stable plasma levels of other antidepressants
- reports of increased or decreased lithium levels when used concomitantly
- elevation of blood levels of certain antipsychotics and anticonvulsants
- caution advised with concomitant administration of CNS active drugs
Contraindications
- contraindicated in patients known to be hypersensitive to it
- not to be used in combination with MAOIs or within 14 days of discontinuing therapy with MAOI
- at least 5 weeks should be allowed after stopping before starting MAOI
Side Effects
- gastrointestinal - nausea, vomiting, dyspepsia, dry mouth, diarrhoea
- neurological - anxiety, nervousness, insomnia, drowsiness, fatigue
- others - excessive sweating, pruritus, skin rashes associated with liver, kidney, and lung involvement
Pregnancy & Lactation
- use in pregnancy only if clearly needed
- caution should be exercised when administered to nursing women
Precautions & Warnings
- use with caution in patients with impaired hepatic or renal function
- use with caution in patients with epilepsy or a history of such disorders
- caution warranted in diabetic subjects
- careful supervision of depressed patients with suicidal tendencies
- not usually considered a suitable form of therapy for the depressive component of bipolar illness
