Pharmacology
- Inhibition of CNS neuronal uptake of serotonin
- Blocking the reuptake of serotonin at the serotonin reuptake pump
- Enhancing the actions of serotonin on 5HT1A autoreceptors
- Virtually no affinity for α1- or α2-adrenergic, β-adrenergic, muscarinic, dopamine D2, histamine H1, GABA-benzodiazepine, opiate, 5-HT1, or 5-HT2 receptors
- Affinity for binding to σ1 receptors
Dosage & Administration
- Recommended starting dose for adults: 50 mg
- Recommended starting dose for pediatric populations (ages 8-17 years): 25 mg
- Maximum therapeutic benefit not to exceed 300 mg per day for adults
- Maximum dose in children up to age 11 should not exceed 200 mg/day
- Elderly patients and those with hepatic impairment have decreased clearance of fluvoxamine maleate
Interactions
- Inhibits several cytochrome P450 isoenzymes (CYP1A2, CYP2C9, CYP3A4, and CYP2C19)
- Elevated carbamazepine levels and symptoms of toxicity with coadministration
- Rare postmarketing reports of weakness, hyperreflexia, and incoordination following use of an SSRI and sumatriptan
- Coadministration with tacrine increased tacrine C max and AUC five- and eight-fold
- Significantly increased plasma TCA levels with tricyclic antidepressants (TCAs)
- Severe vomiting with coadministration of tryptophan
Contraindications
- Coadministration of tizanidine, thioridazine, alosetron, pimozide
- Serotonin Syndrome and MAOIs
- Not to use MAOIs intended to treat psychiatric disorders with Fluvoxamine Maleate Tablets or within 14 days of stopping treatment
Side Effects
- Most common reactions in controlled trials include nausea, somnolence, insomnia, asthenia, nervousness
- Additional events include anorgasmia, decreased libido, dry mouth, and urinary frequency
Pregnancy & Lactation
- Consider both potential risks and benefits when treating a pregnant woman
- Fluvoxamine is secreted in human breast milk
Precautions & Warnings
- Use with caution in patients with a history of mania, seizures, suicide, concomitant ECT, cardiac disease, diabetes mellitus, angle-closure glaucoma, history of bleeding disorders
- May impair the performance of skilled tasks (driving)
