Pharmacological Action
- Potentiation of serotonergic activity
- Inhibition of neuronal reuptake of serotonin
- Selective inhibitor of neuronal serotonin reuptake
- Weak effects on norepinephrine and dopamine neuronal reuptake
- Little affinity for various receptors
Dosage and Administration
- Usual Initial Dosage: 20 mg/day for Major Depressive Disorder
- Maintenance Therapy: Efficacy maintained with average dose of 30 mg for 1 year
- Usual Initial Dosage: 40 mg/day for Obsessive Compulsive Disorder
- Maintenance Therapy: Dosage adjustments for lowest effective dosage
- Usual Initial Dosage: 40 mg/day for Panic Disorder
- Maximum dosage: 60 mg/day
- Usual Initial Dosage: 20 mg/day for Social Anxiety Disorder
- Usual Initial Dosage: 20 mg/day for Generalized Anxiety Disorder
- Usual Initial Dosage: 20 mg/day for Posttraumatic Stress Disorder
Interactions
- Absorption and pharmacokinetics not affected by food or antacids
- Potential interaction with Tryptophan causing serotonin syndrome
- Metabolism affected by drugs which induce or inhibit hepatic drug metabolizing enzymes
- Concomitant use with alcohol not advised
- Potential interaction with MAO inhibitors
- Potential interaction with lithium
- Co-administration with phenytoin associated with decreased plasma concentrations
- Preliminary data suggests pharmacodynamic interaction with warfarin
Contradictions
- Contraindicated in patients taking MAOIs or thioridazine
- Contraindicated in patients with hypersensitivity to Paroxetine

Pregnancy and Lactation

- Paroxetine is secreted in human milk
Precautions and Warnings
- Caution advised when treating patients with cardiac conditions
- Use with caution in patients with epilepsy
- Discontinue in patients who develop seizures
- Little clinical experience of concurrent administration with ECT
- Not associated with impairment of cognitive or psychomotor function
- Taper phase regimen advised for discontinuation of treatment
- Recommended initial dose of 10 mg/day for elderly or debilitated patients
- Dosage should not exceed 40 mg/day

- Recommended initial dose of 10 mg/day for elderly, debilitated, and severe renal or hepatic impairment patients
