Indications

  • Treatment of major depressive disorder

Pharmacology

  • Pharmacodynamics: Enhances central noradrenergic and serotonergic activity, acts as an antagonist at central presynaptic &alpha;2-adrenergic inhibitory autoreceptors and heteroreceptors, is a potent antagonist of 5- HT<sub>2</sub> and 5-HT<sub>3</sub> receptors, and is a moderate peripheral &alpha;1-adrenergic antagonist.
  • Pharmacokinetics: Rapid and well-absorbed after oral administration, with a half-life of elimination of 20-40 hours, and extensive metabolism and elimination via urine and feces.

Dosage & Administration

  • Adult dose: Recommended starting dose of 15 mg/day, with an effective range of 15 to 45 mg/day, and adjustments made at intervals of 1 to 2 weeks.
  • Use in children: Not recommended
  • Missed Dose: Should be taken as soon as remembered unless close to the next dose time, in which case the missed dose should be skipped.

Interaction

Clinically significant drug-drug interactions with Monoamine Oxidase Inhibitors (MAOI) & other serotonergic drugs, may interrupt the metabolism or activity of Carbamazepine, Phenytoin, or Cimetidine, and patients should avoid Alcohol & Diazepam while taking Mirtaz.

Contraindications

  • Hypersensitivity: Contraindicated in patients with known hypersensitivity to Mirtazapine or its excipients.
  • Monoamine Oxidase Inhibitors: Concomitant use with MAO inhibitors is contraindicated.

Pregnancy & Lactation

Pregnancy Category-C, patients should notify their physician if pregnant or breastfeeding.

Precautions & Warnings

  • Behavioral Changes: Patients, families, and caregivers should be alert to the emergence of certain behavioral changes, and patients should be warned about the risk of developing agranulocytosis.
  • Impairment: May impair judgment, thinking, motor skills, and may cause clinically significant ALT (SGPT) elevations.
