Indications

  • Treatment of major depressive disorder (MDD)

Pharmacology

  • Unknown mechanism of action
  • Enhances central noradrenergic and serotonergic activity
  • Acts as antagonist at central presynaptic α2-adrenergic inhibitory autoreceptors and heteroreceptors
  • Potent antagonist of 5- HT2 and 5-HT3 receptors
  • Potent antagonist of histamine (H1) receptors
  • Moderate peripheral α1-adrenergic antagonist
  • Moderate antagonist at muscarinic receptors
  • Rapid and well-absorbed after oral administration
  • Approximately 85% binding to plasma proteins
  • Mean half-life of elimination is 20-40 hours
  • Extensively metabolized and eliminated via the urine and faeces

Dosage & Administration

  • Recommended starting dose of 15 mg/day, administered in a single dose
  • Effective dose range of 15 to 45 mg/day
  • Elimination half-life of approximately 20 to 40 hours
  • Use in children not recommended
  • Guidelines for missed doses

Interaction

  • Clinically significant drug-drug interactions with MAOI & other serotonergic drugs
  • May interrupt the metabolism or activity of Carbamazepine, Phenytoin or Cimetidine
  • Avoid Alcohol & Diazepam

Contraindications

  • Contraindicated in known hypersensitivity to Mirtazapine or to any excipients
  • Concomitant use with MAO inhibitors contraindicated
  • Should not be used within 14 days of initiating or discontinuing therapy with MAOI

  • Patients advised to notify physician if pregnant or intending to become pregnant

  • Alert to emergence of certain behaviors and changes in behavior
  • Warned about risk of developing agranulocytosis
  • May impair judgment, thinking, and motor skills
  • Clinically significant ALT (SGPT) elevations may occur
