Indications

  • Treatment of major depressive disorder (MDD)

Pharmacology

  • Enhances central noradrenergic and serotonergic activity
  • Acts as an antagonist at central presynaptic α2-adrenergic inhibitory autoreceptors
  • Potent antagonist of 5-HT<sub>2</sub> and 5-HT<sub>3</sub> receptors
  • Moderate peripheral &alpha;1-adrenergic antagonist
  • Moderate antagonist at muscarinic receptors
  • Potent antagonist of histamine (H<sub>1</sub>) receptors
  • Rapid and well-absorbed after oral administration
  • Mean half-life of elimination is 20-40 hours
  • Extensively metabolized and eliminated via urine and faeces
  • Displays linear pharmacokinetics within the recommended dose range

Dosage & Administration

  • Recommended starting dose of 15 mg/day
  • Administered in a single dose, preferably in the evening or prior to sleep
  • Effective dose range of 15 to 45 mg/day
  • Missed dose instructions

Interaction

  • Clinically significant drug-drug interactions with Monoamine Oxidase Inhibitors (MAOI) and other serotonergic drugs
  • May interrupt the metabolism or activity of Carbamazepine, Phenytoin, or Cimetidine
  • Patient should avoid Alcohol &amp; Diazepam

Contraindications

  • Contraindicated in patients with hypersensitivity to Mirtazapine or its excipients
  • Concomitant use with a monoamine oxidase (MAO) inhibitor is contraindicated
  • Should not be used within 14 days of initiating or discontinuing therapy with a MAOI

Pregnancy & Lactation

  • Pregnancy Category-C
  • Patients should notify physician if they become pregnant or intend to become pregnant during therapy
  • Patients should notify physician if they are breastfeeding an infant

Precautions & Warnings

  • Patients, their families, and their caregivers should be alert to the emergence of various psychological and behavioral changes
  • Patients should be warned about the risk of developing agranulocytosis
  • May impair judgment, thinking, and motor skills
  • Clinically significant ALT (SGPT) elevations may occur
