Indications

  • Treatment of major depressive disorder (MDD)

Pharmacology

  • Enhances central noradrenergic and serotonergic activity
  • Acts as an antagonist at central presynaptic α2-adrenergic inhibitory autoreceptors and heteroreceptors
  • Antagonist of 5-HT<sub>2</sub> and 5-HT<sub>3</sub> receptors
  • Potent antagonist of histamine (H<sub>1</sub>) receptors
  • Moderate peripheral &alpha;1-adrenergic antagonist
  • Moderate antagonist at muscarinic receptors
  • Rapid and well-absorbed after oral administration
  • Mean half-life of elimination is 20-40 hours
  • Displays linear pharmacokinetics within the recommended dose range

Dosage & Administration

  • Recommended starting dose of 15 mg/day
  • Effective dose range of 15 to 45 mg/day
  • Elimination half-life of approximately 20 to 40 hours
  • Dose changes should be made at intervals of less than 1 to 2 weeks

Interaction

  • Clinically significant interactions with MAOIs, tryptophan, triptans, linezolid, serotonin reuptake inhibitors, venlafaxine, lithium, tramadol, St. John's wort, carbamazepine, phenytoin, cimetidine
  • Avoid concomitant use with alcohol and diazepam

Contraindications

  • Contraindicated in patients with hypersensitivity to Mirtazapine or any excipients
  • Concomitant use with MAOIs is contraindicated
  • Should not be used within 14 days of initiating or discontinuing therapy with an MAOI

  • Be alert to the emergence of various behavioral changes
  • Warned about the risk of developing agranulocytosis
  • May impair judgment, thinking, and motor skills
  • Clinically significant ALT (SGPT) elevations may occur
