Indications
- Treatment of major depressive disorder (MDD)
Pharmacology
- Unknown mechanism of action
- Enhances central noradrenergic and serotonergic activity
- Acts as an antagonist at central presynaptic α2-adrenergic inhibitory autoreceptors and heteroreceptors
- Potent antagonist of 5-HT2 and 5-HT3 receptors
- Moderate peripheral α1-adrenergic antagonist
- Moderate antagonist at muscarinic receptors
- Potent antagonist of histamine (H1) receptors
- Rapid and well-absorbed after oral administration
- Half-life of elimination is 20-40 hours
- Extensively metabolized and eliminated via urine and feces
Dosage & Administration
- Recommended starting dose is 15 mg/day
- Effective dose range is 15 to 45 mg/day
- Once-a-day dosing
- Recommended administration in the evening or prior to sleep
- Dose changes should be made at intervals of less than 1 to 2 weeks
- Not recommended for use in children
Interaction
- Clinically significant interactions with Monoamine Oxidase Inhibitors (MAOI) and other serotonergic drugs
- May interrupt the metabolism or activity of Carbamazepine, Phenytoin, or Cimetidine
- Patients should avoid Alcohol and Diazepam while taking the medication
Contraindications
- Contraindicated in patients with known hypersensitivity to Mirtazapine or its excipients
- Contraindicated with concomitant use of a monoamine oxidase (MAO) inhibitor
- Should not be used within 14 days of initiating or discontinuing therapy with a MAOI
Side Effects
- Common side effects include dizziness, drowsiness, dry mouth, increased appetite, and weight gain
Pregnancy & Lactation
- Pregnancy Category-C
- Patients should notify their physician if they become pregnant or intend to become pregnant during therapy
- Patients should notify their physician if they are breastfeeding
Precautions & Warnings
- Patients, families, and caregivers should be alert to the emergence of various behavioral changes and suicidal ideation
- Patients should be warned about the risk of developing agranulocytosis
- May impair judgment, thinking, and motor skills due to sedative effect
- Clinically significant ALT (SGPT) elevations may occur
