Indications

  • Treatment of major depressive disorder (MDD)

Pharmacology

  • Enhances central noradrenergic and serotonergic activity
  • Acts as an antagonist at central presynaptic α2-adrenergic inhibitory autoreceptors and heteroreceptors
  • Antagonist of 5-HT2 and 5-HT3 receptors
  • Potent antagonist of histamine (H1) receptors
  • Moderate peripheral α1-adrenergic antagonist
  • Moderate antagonist at muscarinic receptors
  • Rapid and well-absorbed after oral administration
  • Half-life of elimination is 20-40 hours
  • Extensively metabolized and eliminated via urine and faeces

Dosage & Administration

  • Recommended starting dose: 15 mg/day
  • Effective dose range: 15 to 45 mg/day
  • Adult dose administered in a single dose, preferably in the evening or prior to sleep
  • Dose changes should be made at intervals of less than 1 to 2 weeks
  • Use in children not recommended
  • Guidelines for missed dose

Interactions

  • Clinically significant drug-drug interactions with MAOIs & other serotonergic drugs
  • May interrupt the metabolism or activity of certain drugs
  • Patient should avoid alcohol & diazepam while taking Rapine

Contraindications

  • Contraindicated in patients with hypersensitivity to Mirtazapine or any of the excipients
  • Concomitant use with MAO inhibitors is contraindicated
  • Should not be used within 14 days of initiating or discontinuing therapy with an MAOI

Pregnancy & Lactation

  • Pregnancy Category-C
  • Patients should notify their physician if they become pregnant or intend to become pregnant during therapy
  • Patients should notify their physician if they are breastfeeding an infant

Precautions & Warnings

  • Patients, families, and caregivers should be alert to the emergence of certain behaviors
  • Patients should be warned about the risk of developing agranulocytosis
  • May impair judgment, thinking, and motor skills
  • Clinically significant ALT (SGPT) elevations may occur
