Indications
- Treatment of major depressive disorder (MDD)
Pharmacology
- Enhances central noradrenergic and serotonergic activity
- Acts as an antagonist at central presynaptic α2-adrenergic inhibitory autoreceptors and heteroreceptors
- Antagonist of 5-HT2 and 5-HT3 receptors
- Potent antagonist of histamine (H1) receptors
- Moderate peripheral α1-adrenergic antagonist
- Moderate antagonist at muscarinic receptors
- Rapid and well-absorbed after oral administration
- Half-life of elimination is 20-40 hours
- Extensively metabolized and eliminated via urine and faeces
Dosage & Administration
- Recommended starting dose: 15 mg/day
- Effective dose range: 15 to 45 mg/day
- Adult dose administered in a single dose, preferably in the evening or prior to sleep
- Dose changes should be made at intervals of less than 1 to 2 weeks
- Use in children not recommended
- Guidelines for missed dose
Interactions
- Clinically significant drug-drug interactions with MAOIs & other serotonergic drugs
- May interrupt the metabolism or activity of certain drugs
- Patient should avoid alcohol & diazepam while taking Rapine
Contraindications
- Contraindicated in patients with hypersensitivity to Mirtazapine or any of the excipients
- Concomitant use with MAO inhibitors is contraindicated
- Should not be used within 14 days of initiating or discontinuing therapy with an MAOI

Pregnancy & Lactation
- Pregnancy Category-C
- Patients should notify their physician if they become pregnant or intend to become pregnant during therapy
- Patients should notify their physician if they are breastfeeding an infant
Precautions & Warnings
- Patients, families, and caregivers should be alert to the emergence of certain behaviors
- Patients should be warned about the risk of developing agranulocytosis
- May impair judgment, thinking, and motor skills
- Clinically significant ALT (SGPT) elevations may occur
