- Treatment of major depressive disorder (MDD)
Pharmacology
- Unknown mechanism of action for treating MDD
- Enhances central noradrenergic and serotonergic activity
- Antagonist at central presynaptic α2-adrenergic inhibitory autoreceptors and heteroreceptors
- Potent antagonist of 5- HT<sub>2</sub> and 5-HT<sub>3</sub> receptors
- Moderate peripheral α1-adrenergic antagonist
- Moderate antagonist at muscarinic receptors
- Sedative effects due to histamine (H<sub>1</sub>) receptors antagonism
- Extensively metabolized and eliminated via urine and faeces
Dosage & Administration
- Recommended starting dose of 15 mg/day
- Effective dose range of 15 to 45 mg/day
- Elimination half-life of 20 to 40 hours
- Avoid use in children
- Instructions for missed dose
Interaction
- Clinically significant drug-drug interactions with MAOI & other serotonergic drugs
- May interrupt the metabolism or activity of Carbamazepine, Phenytoin or Cimetidine
- Patient should avoid Alcohol & Diazepam while taking Rapine
Contraindications
- Contraindicated in patients with known hypersensitivity to Mirtazapine or excipients
- Concomitant use with MAO inhibitors is contraindicated

Pregnancy & Lactation
- Pregnancy Category-C
- Patients advised to notify physician if pregnant or intend to become pregnant during therapy
- Patients advised to notify physician if breastfeeding
Precautions & Warnings
- Patient, families, and caregivers should be alert to the emergence of certain neuropsychiatric events
- Clinically significant ALT (SGPT) elevations may occur
- Rapine may impair judgment, thinking, and motor skills
