Indications

  • Treatment of major depressive disorder (MDD)

Pharmacology

  • Enhances central noradrenergic and serotonergic activity
  • Antagonist at central presynaptic α2-adrenergic inhibitory autoreceptors and heteroreceptors
  • Enhances central noradrenergic and serotonergic activity
  • Potent antagonist of 5- HT<sub>2</sub> and 5-HT<sub>3</sub> receptors
  • Moderate peripheral &alpha;1-adrenergic antagonist
  • Moderate antagonist at muscarinic receptors
  • Potent antagonist of histamine (H<sub>1</sub>) receptors
  • Displays linear pharmacokinetics
  • Metabolized and eliminated via urine and faeces
  • Binding of mirtazapine to plasma proteins is approximately 85%
  • Mean half-life of elimination is 20-40 hours
  • Well-absorbed after oral administration

Dosage & Administration

  • Recommended starting dose: 15 mg/day
  • Effective dose range: 15 to 45 mg/day
  • Elimination half-life: approximately 20 to 40 hours
  • Dose changes should be made at intervals of less than 1 to 2 weeks
  • Use in children not recommended
  • Missed dose instructions

Interaction

  • Clinically significant drug-drug interactions with Monoamine Oxidase Inhibitors (MAOI)
  • Interaction with other serotonergic drugs such as tryptophan, triptans, linezolid, serotonin reuptake inhibitors, venlafaxine, lithium, tramadol, or St. John's wort
  • May interrupt the metabolism or activity of Carbamazepine, Phenytoin or Cimetidine
  • Patient should avoid Alcohol & Diazepam while taking

Contraindications

  • Contraindicated in patients with known hypersensitivity to Mirtazapine or to any of the excipients
  • Concomitant use with monoamine oxidase (MAO) inhibitor

Pregnancy & Lactation

  • Pregnancy Category-C
  • Patients should be advised to notify their physician if they become pregnant or intend to become pregnant during therapy
  • Patients should be advised to notify their physician if they are breastfeeding

Precautions & Warnings

  • Be alert to the emergence of anxiety, agitation, panic attacks, insomnia, irritability, hostility, aggressiveness, impulsivity, akathisia, hypomania, mania, and other unusual changes in behavior
  • Warned about the risk of developing agranulocytosis
  • May impair judgment, thinking, and motor skills
  • Clinically significant ALT (SGPT) elevations may occur
