Indications
- Treatment of major depressive disorder
- Efficacy in 6-week controlled trials for adult outpatients
- Diagnosis using DSM-III or DSM-III-R category of major depression
- Symptoms of major depressive episode
- Maintaining antidepressant response for up to 26 weeks
- Efficacy in recurrent depression for up to 52 weeks
- Long-term evaluation of drug usefulness
Pharmacology
- Mechanism of action unknown
- Potentiation of neurotransmitter activity in the CNS
- Inhibition of serotonin and norepinephrine reuptake
- Weak inhibitory effects on dopamine reuptake
- Not active at histaminergic, muscarinic, or alpha(1)-adrenergic receptors
Dosage & Administration
- Recommended starting dose of 75 mg/day
- Dose increments of up to 75 mg/day
- Maximum dose of 375 mg/day for more severely depressed inpatients
- No dose adjustment recommended for elderly patients
- Several months of sustained pharmacological therapy beyond acute episode
- Careful dosage individualization for elderly patients
- Symptoms associated with discontinuation and gradual reduction in dose recommended
Interaction
- Increased risk of serotonin syndrome with certain medications
- May increase serum levels with CYP3A4 inhibitors
- May increase serum levels of haloperidol
- May decrease serum levels of indinavir
- May increase bleeding risk with certain medications
Contraindications
- Concomitant use with MAOIs or within 14 days of discontinuing MAOI
- Use with linezolid or IV methylene blue

Pregnancy & Lactation

Precautions & Warnings
- History of MI or unstable cardiac disease
- Seizure
- Hypomania or mania
- Increased intraocular pressure or at risk of acute narrow-angle glaucoma
- Renal and hepatic impairment
- Gradual dose reduction recommended
- Impaired ability to drive or operate machinery
- Caution in treating the elderly
- Caution in patients at risk of bleeding
- Pregnancy and lactation considerations
Use in Special Populations
- 50% reduction in total daily dose for mild to moderate hepatic impairment
- Individualization of dosing for patients with cirrhosis
- 25% reduction in total daily dose for mild to moderate renal impairment
- 50% reduction in total daily dose for patients undergoing hemodialysis
Overdose Effects
- Symptoms including sweating, dizziness, somnolence, convulsions
- Management with symptomatic and supportive treatment
- Ensure adequate airway, oxygenation, and ventilation
- Initiate gastric lavage immediately after ingestion
- Administer activated charcoal to reduce absorption
- Monitor cardiac rhythm and vital signs
