Indications

• Treatment of major depressive disorder
• Efficacy in adult outpatients and inpatients
• Symptoms of major depressive episode
• Long-term usefulness evaluation

Pharmacology

• Mechanism of action unknown
• Potentiation of neurotransmitter activity in CNS
• Inhibition of serotonin and norepinephrine reuptake
• Weak inhibitory effects on dopamine reuptake

Dosage & Administration

• Recommended starting dose of 75 mg/day
• Dosage increments
• Dosing for elderly patients
• Maintenance treatment
• Discontinuation symptoms and management

Interaction

• Increased risk of serotonin syndrome
• Effects with other medications like TCA, SSRI, SNRI, lithium, sibutramine, tramadol, CYP3A4 inhibitors, haloperidol
• Bleeding risk with certain medications

Contraindications

• Concomitant use with MAOIs or within 14 days
• Use with linezolid or IV methylene blue

Side Effects

• Changes in behavior
• Suicidal ideation
• Physical symptoms like nausea, headache, insomnia, dry mouth, constipation, dizziness, etc.
• Potential impact on cholesterol and platelet aggregation

Pregnancy & Lactation

• Complications in neonates exposed to the drug in late third trimester

Precautions & Warnings

• Special considerations for patients with history of MI, unstable cardiac disease, seizure, hypomania, mania, increased intraocular pressure or at risk of acute narrow-angle glaucoma, at risk of bleeding, renal and hepatic impairment
• Caution in gradual dose reduction
• Special considerations for pregnancy and lactation
• Impact on ability to drive or operate machinery

Use in Special Populations

• Dosage adjustments for patients with hepatic impairment
• Dosage adjustments for patients with renal impairment

Overdose Effects

• Symptoms like sweating, dizziness, convulsions, cardiac arrhythmias
• Symptomatic and supportive treatment
• Gastric lavage, activated charcoal
• Monitoring of cardiac rhythm and vital signs
