Pharmacology
- Phenothiazine derivative
- Blocks mesolimbic dopaminergic receptors
- Strong α-adrenergic blocking effect
- Competes with histamine for H1-receptor
- Muscarinic blocking effect
- Reduces stimuli to brainstem reticular system
- Well absorbed from gastrointestinal tract
- Clinical effects within 20 minutes
- Lasts 4-6 hours
- Metabolized by the liver
Dosage & Administration
- Initial dose: 25 mg tablet at night
- Maximum 75 mg per day for adults
- Elixir for children
- Antihistamine dosage for children: 5-25 mg
- Sedative dosage for children: 15-25 mg
Interactions
- Enhances anticholinergic agents
- Interferes with immunologic urine pregnancy tests
- Inhibits cutaneous histamine response
- Interaction with tricyclic antidepressants
- Interaction with sedatives/hypnotics
- Avoid alcohol during treatment
Contraindications
- Children under 2 years old
- Hypersensitivity to promethazine
- Coma or CNS depression
- Recent monoamine oxidase inhibitor use

- Avoid in Reye’s syndrome symptoms
- May thicken/dry lung secretions
- Caution in asthma/bronchitis/bronchiectasis

- Use with care in epilepsy
- Use with care in hepatic/renal insufficiency
- Caution in bladder neck obstruction

Overdose Effects
- Variable symptoms in severe overdose
- Excitation, ataxia, hallucinations in children
- Drowsiness, coma in adults
- Convulsions may occur
- Possible tachycardia
- Cardiorespiratory depression uncommon
- Induce vomiting if seen soon after ingestion
- Supportive treatment for adequate respiratory/circulatory status
- Treat convulsions with suitable anticonvulsant
