Phenerex 50 mg/2 ml (Injection)

Indications

• Amelioration of allergic reactions to blood or plasma
• Adjunct to epinephrine in anaphylaxis
• Treatment of uncomplicated allergic conditions
• Sedation and relief of apprehension
• Active treatment of motion sickness
• Prevention and control of nausea and vomiting
• Adjunct to analgesics for postoperative pain control
• Preoperative, postoperative, and obstetric sedation
• Special surgical situations such as bronchoscopy and ophthalmic surgery

Pharmacology

Blocks postsynaptic mesolimbic dopaminergic receptors, exhibits α-adrenergic blocking effect, competes with histamine for the H1-receptor

Dosage & Administration

• Average adult dose is 25 mg for allergic conditions
• Nighttime sedation may be achieved with 25 to 50 mg for hospitalized adult patients
• Usual adult dose for nausea and vomiting is 12.5 to 25 mg
• Adjunct to preoperative or postoperative medication is 25 to 50 mg for adults

Interaction

• May intensify the sedative action of CNS depressants
• Epinephrine should not be used in case of Phenerex injection overdose

Contraindications

• Contraindicated in comatose states
• Should not be given by intra-arterial or subcutaneous route

Side Effects

• Drowsiness
• Extrapyramidal reactions
• Tachycardia, bradycardia
• Nausea, vomiting
• Allergic reactions like urticaria and dermatitis
• Leukopenia and agranulocytosis
• Thrombocytopenic purpura and jaundice
• Tissue necrosis with subcutaneous injection

Pregnancy & Lactation

Pregnancy Category C, should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus

Precautions & Warnings

• Use with caution in patients with narrow-angle glaucoma
• No long term animal studies for carcinogenic potential, mutagenicity, or impairment of fertility

Use in Special Populations

• Contraindicated for children under 2
• Dosage should not exceed half the suggested adult dose in pediatric patients
• Dosage should be reduced for geriatric patients

Overdose Effects

• Range from mild depression to profound hypotension and unconsciousness
• Convulsions may rarely occur
• Treatment is essentially symptomatic and supportive
