Indications
- Relief of symptoms in adults with acute and recurrent diabetic gastroparesis

Pharmacology
- Blocks dopamine receptors
- Blocks serotonin receptors in chemoreceptor trigger zone of the CNS
- Enhances the response to acetylcholine of tissue in the upper GI tract
- Accelerates gastric emptying without stimulating gastric, biliary, or pancreatic secretions
- Increases lower esophageal sphincter tone
Dosage & Administration
- Adults less than 65 years of age: Recommended dosage is 1 spray (15 mg) in one nostril, 30 minutes before each meal and at bedtime (max of 4 sprays daily) for 2 to 8 weeks
- Adults 65 years of age and older: Metoclopramide is not recommended as initial therapy, but can be used as an alternative at a stable dosage
- Continuous administration: 10 to 15 mg, 30 minutes before each meal and at bedtime (max of 60 mg per day) for 4 to 12 weeks
- Intermittent administration: Single doses up to 20 mg prior to provoking situation
- Diabetic Gastroparesis: 10 mg, 30 minutes before each meal and at bedtime (max of 40 mg per day) for 2 to 8 weeks
Contraindications
- Patients with a history of tardive dyskinesia or a dystonic reaction to metoclopramide
- When stimulation of gastrointestinal motility might be dangerous
- In patients with pheochromocytoma or other catecholamine-releasing paragangliomas
- In patients with epilepsy
- In patients with hypersensitivity to metoclopramide
Side Effects
- Most common adverse reactions (≥5%) are: dysgeusia, headache, and fatigue
Pregnancy & Lactation
- Pregnancy Category B: Reproduction studies demonstrated no impairment of fertility or significant harm to the fetus due to metoclopramide, but there are no adequate and well-controlled studies in pregnant women
- Excreted in human milk, caution should be exercised when administered to a nursing mother
Precautions & Warnings
- Avoid concomitant use of other drugs known to cause TD/EPS/NMS
- Avoid use in patients with Parkinson’s Disease
- Discontinue if symptoms of TD, EPS, or NMS occur
- Avoid use in patients with depression and suicidal ideation/suicide
