Indications
- Relief of symptoms in adults with acute and recurrent diabetic gastroparesis
- Treatment of symptomatic, documented gastroesophageal reflux in adults who fail to respond to conventional therapy
- Prevention of nausea and vomiting associated with emetogenic cancer chemotherapy
- Relief of symptoms associated with acute and recurrent diabetic gastric stasis
Pharmacology
- Metoclopramide blocks dopamine receptors
- It blocks serotonin receptors in the chemoreceptor trigger zone of the CNS
- Enhances the response to acetylcholine of tissue in the upper GI tract
- Accelerates gastric emptying without stimulating gastric, biliary, or pancreatic secretions
- Increases lower esophageal sphincter tone
Dosage & Administration
- Recommended dosage of 1 spray (15 mg) in one nostril for adults less than 65 years of age
- Maximum of 4 sprays daily for 2 to 8 weeks, depending on symptomatic response
- Administer 10 to 15 mg continuously or intermittently for gastroesophageal reflux
- Administer 10 mg for acute and recurrent diabetic gastroparesis
- Dosage for prevention of nausea and vomiting associated with emetogenic cancer chemotherapy
- Usual adult dose for prevention of postoperative nausea and vomiting is 10 mg
- Intravenous infusions should be made slowly over a period of not less than 15 minutes for cancer chemotherapy
Contraindications
- Patients with a history of tardive dyskinesia (TD) or a dystonic reaction to metoclopramide
- Stimulation of gastrointestinal motility might be dangerous in the presence of gastrointestinal hemorrhage, mechanical obstruction, or perforation
- Patients with pheochromocytoma or other catecholamine-releasing paragangliomas
- Patients with epilepsy
- Patients with hypersensitivity to metoclopramide
Side Effects

Pregnancy & Lactation
- Pregnancy Category B
- Reproduction studies performed in rats, mice and rabbits have demonstrated no impairment of fertility or significant harm to the fetus due to metoclopramide
- No adequate and well-controlled studies in pregnant women
- The drug should be used during pregnancy only if clearly needed
- Metoclopramide is excreted in human milk
- Caution should be exercised when metoclopramide is administered to a nursing mother
Precautions & Warnings
- Avoid concomitant use of other drugs known to cause TD/EPS/NMS
- Depression and suicidal ideation/suicide: Avoid use
