Indications
- Relief of symptoms in adults with acute and recurrent diabetic gastroparesis
- Treatment for 4 to 12 weeks of symptomatic, documented gastroesophageal reflux in adults who fail to respond to conventional therapy
- Prevention of nausea and vomiting associated with emetogenic cancer chemotherapy
- Prevention of postoperative nausea and vomiting
- Facilitates small bowel intubation in adults and pediatric patients
- Stimulates gastric emptying and intestinal transit of barium for radiological examination
Pharmacology
- Blocks dopamine receptors
- Blocks serotonin receptors in chemoreceptor trigger zone of the CNS
- Enhances the response to acetylcholine of tissue in upper GI tract
- Accelerates gastric emptying without stimulating gastric, biliary, or pancreatic secretions
- Increases lower esophageal sphincter tone
Dosage & Administration
- Recommended dosage of 1 spray (15 mg) in one nostril, 30 minutes before each meal and at bedtime (maximum of 4 sprays daily) for 2 to 8 weeks
- Alternate dosage for adults 65 years of age and older
- Continuous administration of 10 to 15 mg, 30 minutes before each meal and at bedtime (maximum of 60 mg per day) for 4 to 12 weeks
- Intermittent administration of up to 20 mg prior to provoking situation
- Recommended dosage of 10 mg, 30 minutes before each meal and at bedtime (maximum of 40 mg per day) for 2 to 8 weeks
- Intravenous infusions made slowly over a period of not less than 15 minutes
- Intramuscular administration near the end of surgery
Contraindications
- History of tardive dyskinesia (TD) or a dystonic reaction to metoclopramide
- Stimulation of gastrointestinal motility might be dangerous
- Presence of gastrointestinal hemorrhage, mechanical obstruction, or perforation
- Presence of pheochromocytoma or other catecholamine-releasing paragangliomas
- Epilepsy
- Hypersensitivity to metoclopramide

Pregnancy & Lactation
- Pregnancy Category B
- No impairment of fertility or significant harm to the fetus demonstrated in animal studies
- Excretion in human milk
- Caution in nursing mothers
Precautions & Warnings
- Avoid concomitant use of other drugs known to cause TD/EPS/NMS
- Avoid use in patients with Parkinson’s Disease
- Discontinue and seek immediate medical attention if TD/EPS/NMS symptoms occur
- Avoid use in depression and suicidal ideation/suicide
