Indications
- Treatment of hypertension
- May be used alone or in combination with other antihypertensive agents
Pharmacology
- Blocks vasoconstrictor effects of angiotensin II
- Selective binding of angiotensin II to AT 1 receptor in vascular smooth muscle
- Does not inhibit ACE (kininase II)
Dosage & Administration
- Usual recommended starting dose of 20 mg once daily
- Daily dosage may be increased to 40 mg if further reduction in blood pressure is required
- No initial dosage adjustment recommended for elderly patients or those with moderate to marked renal/hepatic impairment
Interaction
- No significant drug interactions reported with digoxin or warfarin
- Bioavailability not significantly altered by antacids
- Potential renal function deterioration when co-administered with NSAIDs
Contraindications
- Contraindicated in patients hypersensitive to any component of the product
Side Effects
- Well tolerated with incidence of adverse reactions similar to placebo
- Dizziness, back pain, increased creatine phosphokinase, diarrhea, headache, hyperglycemia, among the reported adverse reactions
Pregnancy & Lactation
- Discontinue if pregnancy is detected
- Drugs like Olmesartan can cause injury to developing fetus when used during second and third trimesters
- Potential excretion in human milk with adverse effects on the nursing infant
Precautions & Warnings
- Inhibiting the renin-angiotensin-aldosterone system may lead to changes in renal function
- Oliguria, progressive azotemia, acute renal failure, and death are possible in susceptible individuals
Overdose Effects
- Likely effects include hypotension and tachycardia
- Clinically significant hypotension requires active support of the cardiovascular system
