Indications
- Prevention of nausea and vomiting associated with initial and repeat courses of emetogenic cancer chemotherapy

Description
- Orally dissolving film designed to be applied on top of the tongue
- Dissolves within 20 seconds
- Active ingredient: Ondansetron base
- Empirical formula: C18H19N3O

Pharmacology
- Potent and highly selective 5HT3 receptor-antagonist
- Precise mode of action in the control of nausea and vomiting is not known
- Blocks initiation of vomiting reflex by activating vagal afferents via 5HT3 receptors
- Antagonizes 5HT3 receptors on neurons located both in the peripheral and central nervous system
Dosage
- Different doses for adults, pediatric patients, and radiotherapy
- Oral soluble film dose varies for different conditions and age groups
- Injection doses are based on patient weight and specific chemotherapy/radiotherapy conditions
Administration
- Tear the pouch carefully along the edge tear mark
- Place the Ondansetron film on top of your tongue
- Do not chew or swallow the film whole
- Swallow after the film dissolves
- Wash hands after taking the film
Interaction
- Metabolized by hepatic cytochrome P-450 drug-metabolizing enzymes
- Inducers or inhibitors of these enzymes may change clearance and half-life
- No dosage adjustment recommended for patients on these drugs
Contraindications
- Hypersensitivity to the drug or any of its components
- Concomitant use of apomorphine
Side Effects
- Frequently reported adverse events: headache, constipation, diarrhea
- Mild or moderate in nature
- Rash reported in chemotherapy-induced nausea and vomiting patients
- Rare cases of anaphylaxis, bronchospasm, tachycardia, chest pain, hypokalemia
- No evidence of extrapyramidal reactions
Pregnancy & Lactation
- No evidence of impaired fertility or harm to fetus in animal studies
- No adequate and well-controlled studies in pregnant women
- Excreted in the breast milk of rats
- Caution advised when administered to nursing women
Precautions & Warnings
- Hypersensitivity reactions reported in patients with hypersensitivity to other selective 5-HT3 receptor antagonists
- Not a drug that stimulates gastric or intestinal peristalsis
- Should not be used instead of nasogastric suction
- May mask a progressive ileus and/or gastric distension
Use in Special Populations
- Dosage recommendation same for patients with impaired renal function
- Dosage adjustment for severe hepatic impairment
- Limited information available about dosage in pediatric patients 4 years of age or younger
- No dosage adjustment needed in patients over the age of 65
