Indications
- partial and secondary generalized tonic-clonic seizures
- Primary generalized tonic-clonic seizures
- Trigeminal neuralgia
- Prophylaxis of bipolar disorder
Pharmacology
- Depresses activity in the nucleus ventralis of the thalamus
- Reduces synaptic propagation of excitatory impulses
- Decreases summation of temporal stimulation
- Limits influx of Na ions across cell membrane
- Stimulates the release of antidiuretic hormone (ADH)
- Potentiates the action of ADH in promoting reabsorption of water
Dosage & Administration
- Available in tablets, controlled release tablets, and suspension forms
- Different initial and maintenance dosages based on age groups
- Can be used in combination therapy with other anticonvulsants
- Gradual addition to existing anticonvulsant therapy when used in combination
- Special regimen for trigeminal neuralgia
Interaction
- Potential interaction with oral contraceptives
- Hepatic enzyme inducers may increase metabolism of the medicine
- May require dosage adjustment when used with other drugs
Contraindications
- Allergy to any of the ingredients
- Problems with the electrical message pathways in the heart (atrioventricular block)
- History of decreased blood cell production by the bone marrow (bone marrow depression)
- Hereditary blood disorders called porphyrias
- Allergy to tricyclic antidepressants
- Recent use of monoamine-oxidase inhibitor antidepressants (MAOI) within the last 14 days
Side Effects
- Common side effects including dizziness, drowsiness, ataxia, dry mouth, abdominal pain, nausea, vomiting, anorexia, leucopenia, proteinuria
- Severe adverse reactions in the hemopoietic system, the skin, and the cardiovascular system
- Increased sensitivity to the sun
- Potential erythematous skin rash and aplastic anemia
Pregnancy & Lactation
- Pregnancy category D
- Transfer of the medicine and its metabolite to breast milk
- Need to decide whether to discontinue nursing or discontinue the drug based on potential side effects
Precautions & Warnings
- May cause dizziness and drowsiness
- May cause skin reactions
- May rarely cause liver problems
- Decreases blood levels of hormonal contraceptives
- Should not be stopped suddenly unless advised by a doctor
- Caution in mixed seizures including absence seizures, elderly people, history of heart/kidney/liver disease, raised pressure in the eye, history of blood disorders caused by other medication, and previous Carbazin therapy interruption due to side effects or allergy
